Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver compassionate, holistic home-based care. You will collaborate with a care team to develop personalized plans, lead meaningful end-of-life discussions, and ensure symptom management with empathy and clinical excellence.

You will assess, educate, and coordinate care for patients and families, while maintaining autonomy in practice and participating in on-call coverage. Join our team where your work truly matters.

Qualifications

• 1+ years of acute care experience required; hospice/home health experience preferred.
• Understands end-of-life care and symptom management.
• Strong communication with patients and families.

Responsibilities

• Develop individualized care plans with patients and families.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and medications.
• Rotate on-call duties and support transitions between care settings.
