St. Croix Hospice is seeking a Registered Nurse Case Manager to provide compassionate, in-home hospice care. You will develop personalized plans, lead end-of-life conversations, and coordinate symptom management with the care team.

Requires an RN with a nursing degree and 1+ year acute care experience; hospice/home health experience preferred. Must have reliable transportation and be on-call as part of the schedule.

Qualificações

• RN license required; current nursing license in good standing.
• 1+ year of acute care experience; hospice/home health experience preferred.
• Developing patient-centered care plans and providing education to patients and caregivers.

Responsabilidades

• Assess patients and families to determine hospice needs and plan of care.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed; monitor symptoms.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
