Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide holistic, in-home hospice care. You will lead individualized care plans, conduct comprehensive assessments, and guide patients and families through end-of-life conversations with empathy and clinical excellence.

The role requires an RN with acute care experience, hospice knowledge, and on-call flexibility. A valid driver’s license and reliable transportation are essential for home visits.

Qualifications

• 1+ years of recent acute care experience required; hospice/home health experience preferred.
• Registered Nurse with an accredited nursing program (RN).
• Must have reliable transportation and valid driver's license.

Responsibilities

• Assess patient needs and develop personalized plan of care.
• Deliver medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Coordinate with care team for seamless transitions and on-call coverage.
