Job Overview

We are seeking a dedicated Registered Nurse Case Manager to join our Hospice Home Care team in Asheville with CarePartners Health Services. This role involves providing high-quality, compassionate end-of-life care to patients in a home setting.

Responsibilities

• Follow the plan of care, administer treatments, and collaborate with the healthcare team to ensure positive patient outcomes.
• Provide direct patient care in accordance with the State Nurse Practice Act, including end-of-life care duties.
• Coordinate hospice services, report patient needs to the interdisciplinary team, including Hospice Aide, Director of Clinical Services, and Chaplain.
• Manage pain and symptoms effectively.

Qualifications

• Registered Nurse (RN) license.
• Associate Degree in Nursing.
• Basic Cardiac Life Support (BCLS) certification within 30 days of employment.
• Class C Driver's License.
