Responsibilities

• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the physical, psychosocial, and environmental needs of patients and families at least every 15 days.
• Develop and coordinate the plan of care, maintaining accurate clinical documentation.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate daily with patients, families, physicians, and the interdisciplinary team.

Qualifications
