Job summary

Adoration Hospice in Murfreesboro, TN is hiring a Registered Nurse Case Manager to deliver patient-centered hospice care across Williamson and Rutherford counties. You will assess needs, develop care plans, administer treatments, manage symptoms, and educate families while partnering with an interdisciplinary team.

The role requires TN RN licensure and one year of nursing experience, with hospice familiarity preferred. Monday–Friday schedule with on-call rotation and local travel supported.

Qualifications

• Graduate of an accredited nursing program with RN licensure in Tennessee.
• Minimum 1 year nursing experience; hospice or similar setting preferred.
• Strong clinical and documentation skills with CPR certification.
• Valid driver's license with insured vehicle.
• Commitment to patient- and family-centered hospice care.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs.
• Create and adjust hospice care plans aligned with patient goals.
• Direct nursing care: medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate, timely documentation; participate in on-call rotation.

Schedule: Monday thru Friday, 8am-5pm

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
