Job summary

Adoration Hospice is seeking a dedicated Hospice RN Case Manager to join our Cleveland, MS team. You will conduct patient assessments, develop care plans, administer treatments, and provide compassionate support to patients and their families.

In this full-time role you will collaborate with an interdisciplinary team, ensure thorough documentation, and participate in on-call rotations to ensure continuous, high-quality end-of-life care for our patients in the community.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient’s wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
