Job summary

Adoration Health is seeking an experienced Registered Nurse Case Manager for Hospice to join our Yazoo City, MS team. You will plan and coordinate comprehensive hospice care, ensuring patient wishes guide care decisions.

The role requires MS RN licensure, 1 year minimum nursing experience (3+ preferred) and ability to drive. We offer supportive, team-based care with opportunities for growth and stability.

Qualifications

• Must be licensed to practice in Mississippi.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Auto driver with insured vehicle is required.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care: administer medications and treatments.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team and document promptly.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
