Job summary

Adoration Health is seeking a passionate Hospice RN Case Manager to join our Lawrenceville, GA team. You will provide expert, patient-focused care in a supportive environment, coordinating with an interdisciplinary team to ensure compassionate end-of-life care.

The role emphasizes independent practice with team support, meticulous documentation, and adherence to hospice philosophy. The ideal candidate has RN licensure, at least 1 year of nursing experience (3+ years preferred), and a commitment

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
