Job summary

BrightSpring Health Services, through its Adoration Health affiliate, is seeking a Registered Nurse Case Manager to join our hospice team in Lawrenceville, GA. You will provide compassionate, patient-centered care at a critical time, coordinating with families and interdisciplinary colleagues to develop and monitor individualized care plans.

The role emphasizes on-call availability, thorough documentation, and adherence to hospice philosophy.

Qualifications

• Graduate of an accredited nursing program with current GA licensure.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligned with patient goals.
• Provide direct nursing care: administer medications, treatments, interventions.
• Manage pain and symptoms effectively.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
