Job summary

VNA Health in Santa Barbara, CA seeks a healthcare liaison to provide patient consultation, education and referrals for home health, hospice and palliative services. You will evaluate referrals, coordinate plans of care, and promote VNA Health offerings to physicians and community partners.

You will collaborate with clinicians and case managers, participate in committee and marketing activities, and help grow program reach while ensuring compliance with regulations and high-quality patient care.

Qualifications

  • Current and active clinical California license (RN/LVN/PT/OT).
  • Current CPR certification.
  • Proven ability to develop and maintain positive relationships with referral sources.
  • Ability to promote services and grow the business.
  • Excellent observation and verbal/written communication, and team skills.
  • Ability to work with other health care clinicians on a home plan of care.
  • Minimum one year experience in a health care setting.
  • Understanding of home care, hospice regulations, and insurance eligibility is highly desired.
  • Self-directed and able to work with minimal supervision.
  • Licensed driver with insured automobile.

Responsibilities

  • Facilitate referrals to Home Health, Hospice and Palliative Care with referral sources.
  • Meet or exceed annual budgeted admission goals.
  • Evaluate potential patients and referrals for eligibility for home health, hospice or palliative care.
  • Coordinate with physicians, discharge planners, insurance case managers and others to ensure safe discharge and a coordinated home plan of care.
  • Coordinate referrals to ensure timely response, admission and follow up with referral sources.
  • Interview patients, families and caregivers to assess home needs and plan of care.
  • Identify and educate referral sources and the community about VNA Health services and programs.
  • Attend interdisciplinary team meetings, staff meetings and trainings.
  • Develop and maintain relationships supporting mission and business development.
  • Participate in marketing planning for program growth.
  • Meet productivity standards and adhere to policies of facilities.

Job Description Summary

Under the direction and supervision of the Senior Director of Hospice, provide patient consultation, assistance, and education about VNA Health and its home health, hospice and palliative care services in the hospital. Evaluate, plan and coordinate patient referrals to VNA Health services. Develop and maintain relationships that promote and enhance ongoing business development. Promotes and supports the mission, values and vision of VNA Health.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
  • Function cooperatively with referral sources and Community Liaison team to:
    • Facilitate referrals to Home Health, Hospice and Palliative Care
    • Meet or exceed annual budgeted admission goals.
  • Evaluate potential patients and referrals from all sources for their appropriateness and eligibility for home health, hospice, or palliative care.
  • Identify and provide information regarding insurance coverage and when appropriate, requests prior authorization.
  • Consult and collaborate with physicians, discharge planning personnel, insurance Case Managers and other sources to facilitate safe, appropriate discharge and/or establish a coordinated home plan of care.
  • Coordinate referral with appropriate agency contact to ensure timely response, admission and/or follow up with referral source. Establish and maintain lines of communication between VNA Health and referral sources.
  • Interview the patient, family, and/or caregiver and discusses the home situation, current needs, and any psychosocial factors that are relevant to the plan. Complete referral information that includes intake data, essential background information, hospital course, and the plan of care.
  • Identify, coordinate and/or provide ongoing education to assigned referral sources and the community regarding VNA Health services and programs.
  • Attend and participate in ongoing organization meetings, including but not limited to, interdisciplinary team conferences, staff meetings and in-service training.
  • Complete all referral and activity reports as assigned.
  • Develop and maintain relationships that support and enhance the mission of VNA Health and promote ongoing business development.
  • Participate in the development of annual marketing plan to generate budgeted program growth.
  • Consistently meet or exceed position expectations and productivity standards, as identified at start of employment and thereafter at annual evaluations, or as revised.
  • Follow policies & procedures of community facilities as required.

POSITION QUALIFICATIONS
