What We Are Looking For
- Graduate of an accredited school of professional nursing
- Current license as a Registered Nurse (RN) with the State of MN
- CPR certification
- Demonstrated knowledge of safety and infection control practices
- Excellent organizational skills
- Extraordinary communication skills
- Ability to use technology solutions for scheduling, charting, training and reviewing
- Be involved and team focused. We are looking for someone hungry to help find solutions to challenges presented each day.
- Experience in home care (preferred, not required).
- Experience developing training programs (preferred, not required).
What You Will Be Doing
- Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
- Providing interventions with a focus on achieving realistic client outcomes.
- Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
- Administering medications as needed.
- Managing and delegating tasks to other nurses and caregivers.
- Maintaining client records showing systematic assessments, planning interventions, and evaluations.
- Initiating referrals as needed to other health team members.
- First shift introductions of caregivers to clients.
- Train and engage caregivers to deliver quality, consistent care.
- Operate orientations and training programs for staff.
- Manage QAPI program.
Schedule
- Full-Time or Part-Time
- Monday - Friday
- On Call Rotation for triage, client intros, orientations, assessments (as needed)
Flexible work from home options available.
Compensation: $35.00 - $45.00 per hour
