Job Summary

This role involves primary ownership and oversight of a specific panel of members with varying health statuses and clinical needs. The incumbent assesses health management needs, utilizes data and analytics alongside clinical judgment to identify appropriate interventions, and manages an active case load. Responsibilities include developing care plans, encouraging behavioral changes, addressing barriers, coordinating care, and utilizing resources to meet personal health goals. The role also involves monitoring and improving quality outcomes across clinical, financial, and functional metrics.

Essential Responsibilities

• Oversee a panel of members, assessing their health management needs and referring to appropriate resources.
• Create and implement care plans, set goals, and conduct regular re-assessments.
• Achieve targeted patient outcomes and document activities in compliance with standards and regulations.
• Maintain knowledge of applicable regulations and standards of practice.
• Perform other duties as assigned.

Education

• Required: High School Diploma/GED
• Preferred: Bachelor's Degree in Nursing

Experience

• Required: 7 years of clinical, case management, or related experience.
• Preferred: Experience with cognitive behavioral therapy, motivational interviewing, or dialectical behavior therapy; working with diverse populations; understanding cultural competency.

Licenses and Certifications

• Required: RN license in PA, WV, DE, or NY (other states within 6 months).
• Preferred: Oncology experience, case management certification.

Skills

• Strong communication, negotiation, and interpersonal skills.
• Broad knowledge of disease processes and healthcare systems.
• Proficiency in MS Excel and analytical skills.

Additional Information

Travel: 0-25%

Position: Office-based with occasional travel; physical demands are minimal.

Compensation

Range: $57,700 - $107,800 annually, based on qualifications and experience.
