Job summary

Bristol Hospice in Salem, OR is seeking dedicated RN Case Managers who are passionate about compassionate, whole-person care and serving as advocates for patients and families through every step of this sacred journey.

You will develop and implement individualized care plans, monitor vital signs, educate families, document progress, and collaborate with interdisciplinary teams to ensure comfort, dignity, and high-quality end-of-life care in a community-based setting.

Qualifications

• Active RN license in Oregon.
• CPR certification (current).
• Valid driver’s license and reliable transportation.
• Strong observational and written/verbal communication skills.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs regularly.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
