Provides case management for patients in the acute care setting. In collaboration with the physician and other health care team members, the RN Case Manager coordinates the patient’s care to achieve optimal patient outcomes. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served, including knowledge of physical and psychological needs. Performs case management duties toward targeted patient care and resource utilization goals. Guides the team to provide safe and effective care progression. Identifies patient and family needs related to continuity and efficiency of care. Works with physicians to assess patient needs, develop and implement care plans. Coordinates care for patients with complex needs and facilitates discharge planning. Collaborates with Utilization Review Coordinators to provide necessary payer information. Serves as clinical expert/resource to staff regarding resource utilization and patient care. Participates in clinical improvement activities and mentors staff. Enhances patient satisfaction through communication and problem solving. Maintains clinical expertise through ongoing education and professional involvement. Requires Bachelor’s Degree in Nursing or related field with case management experience, minimum one year case management or clinical specialist experience, Illinois RN licensure, and CPR certification. Preferred certification in case management.
