Baptist Health Downtown Jacksonville is seeking an experienced Registered Nurse, Case Manager to join our In-Patient Case Management Department on a PRN Day shift. The ideal candidate will coordinate care for hospitalized patients, perform high-risk screenings, organize post-acute services, and collaborate with a multidisciplinary team.

Active Florida RN license and ADN required. This role offers the opportunity to contribute to patient-centered care within a faith-based, not-for-profit health
