Job summary

Baptist Health in Jacksonville Downtown is seeking a Registered Nurse, Case Manager for the Full-Time Day shift in the In-Patient Case Management Department.

You will coordinate care for hospitalized patients, perform high-risk screenings, and collaborate with a multidisciplinary team to coordinate post-acute services.

The role requires an ADN, Florida RN license, and BLS certification, with at least 1 year in case management; join a faith-based, not-for-profit system.

Qualifications

• BLS certification required.
