Baptist Health is seeking a Registered Nurse, Case Manager for the Downtown Jacksonville campus. This full-time day shift role in the In-Patient Case Management Department focuses on coordinating care for hospitalized patients and ensuring smooth transitions to post-acute services.

The ideal candidate has 1 year of case management experience, an ADN, and an active Florida RN license with BLS certification. Licensed drivers and a valid DMV may be required to support field coordination as needed.

Qualifications

• 1 year case management experience.
• Associate's Degree in Nursing (ADN).
• Active Florida RN license (RN FL).
• Basic Life Support (BLS) certification.
• Driver's license (may be required).
