Summary

This Registered Nurse Case Manager position is in Nursing - Care in the Community at the Newington VA Healthcare System, located at the Newington campus. This is a full-time position, 40 hours per week.

Qualifications

Basic Requirements:

• Proficiency in English (written and spoken), in accordance with 38 U.S.C. 7403(f).
• Graduate of an accredited school of professional nursing (ACEN or CCNE).
• Or, enrolled in a master's level bridge program with completion of coursework equivalent to a bachelor's degree in Nursing, leading to licensure.
• Or, foreign nursing graduates with current, unrestricted registration and degree validation.
• Current, full, active, and unrestricted nursing license in the U.S. or its territories.
• Graduate Nurse Technician (GNT) exception for candidates pursuing licensure.

Preferred Experience: 5 years of nursing experience, including Case Management.

Physical Requirements: Pre-employment physical and medical exam required. Physical demands include lifting, reaching, finger dexterity, walking, standing, bending, working alone, and working irregular hours with emotional strain.

Duties

• Provide competent, evidence-based care to Veterans in inpatient and outpatient settings.
• Assist Veterans, families, and caregivers with healthcare options and services.
• Coordinate care across multiple providers and levels of care, including managing complex and life-threatening conditions.
• Address psychosocial, nursing, and medical needs through interdisciplinary care management.
• Evaluate care outcomes and ensure timely service delivery.
• Demonstrate leadership in holistic care and collaborative strategies.
