DuMolin Community Living in California seeks a Registered Nurse Case Manager to lead healthcare services for residents in ICF/DD facilities. You will work with an interdisciplinary team to develop care plans, provide nursing education, and support daily operations for 6-12 clients across one or two homes.
Ideal candidates have a CA RN license and at least two years in community health or long-term care, with strong communication and adaptability to changing regulatory demands.
Join us at DuMolin Community Living! Since 1976, we have been dedicated to providing warm and loving residential care to the developmentally disabled community. With many of our team members having been with us for over 15 years, we create a family-like atmosphere where everyone feels valued.
At DuMolin Community Living, we strive to treat each other with kindness, respect, and compassion—just as we expect our team to treat our clients. If you're looking to join a caring team that makes a positive impact every day, we’d love to hear from you!
Join our team and be a part of our mission to provide exceptional care and support to our residents. We currently have full-time positions available at our licensed ICF/DD-N and ICF/DD-H facilities. If you're looking for a fulfilling role where you can make a difference while enjoying yourself, we encourage you to apply.
Residents that live in our homes have a variety of intellectual and developmental disabilities as well as physical impairments. Our homes are licensed through California Department of Public Health and are classified as ICF/DD-N (Intermediate Care Facility/Developmentally Disabled - Nursing) and ICF/DD-H (Intermediate Care Facility/Developmentally Disabled - Habilitative) facilities.
Our residents come and go throughout the day and weekend to attend Day Programs, Work Programs and visit family and friends; some are more independent than others in their outings and we support them whenever we can so they can have free access to their community and community events. We also encourage fully staffed whole house outings to places like the beach, Sonoma County Fair, baseball games, dances, concerts and the like. If our staff is having fun then so are our residents!
The Registered Nurse's major responsibility is the promotion and maintenance of client health through teaching, counseling, and appropriate preventative and habilitative measures. The Registered Nurse Case Manager serves as a member of each client's individual service plan team and provides direct input toward the development of the clients' individual service plan.
Full-Time 20-26 hours a week. Must reside within 25 Santa Rosa, CA
