Coordinates utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation. Oversees acute patient populations across the care continuum to provide coordinated and integrated care, preventing unnecessary admissions or readmissions. Provides discharge planning coordination and intervention for high-risk patients in the Emergency Department. Aims to promote patient wellness, improve care outcomes, utilize health services efficiently, and minimize payment denials for patients with complex health needs.

Shift Details

5x8s 0830-1700, minimum 2 weekend shifts per month, if floated, will only work in CM.
