Home & Health Care Management in Northern California seeks a registered nurse to join an interdisciplinary care management team supervising clients receiving home and community‑based services.
You will conduct monthly visits, review medical records and coordinate with the RN and Social Work Case Managers to ensure high-quality, person-centered care. This full‑time position in Sacramento includes telehealth portions, travel to client homes, and a competitive benefits package.

This role is for a Registered Nurse to work with an interdisciplinary care management team supervising the care of clients who are receiving home and community‑based services.
This role is located in Sacramento serving Sacramento County and surrounding counties.
This is not a remote role and requires you to work in our Sacramento office.
Full time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.
Currently 20% of the time is related to direct home visits, 80% of the time are telehealth video visits and or calls and documentation in the office.
This role involves some travel to and from clients' homes.
Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically includes the ability to lift to 35 pounds without assistance.
PPE is provided to all employees
Pay range is $43 to $53 per hour. Exact compensation may vary based on skills and experience.
