Job summary

Bristol Hospice in Colorado is seeking an experienced RN Case Manager to advocate for patients and families, delivering compassionate, whole-person care. You will develop and implement care plans, and walk with patients through every step of their journey.

You must hold an active RN license in good standing in Colorado, have CPR, a valid driver’s license, and excellent communication skills. This on-site role offers a supportive team and a mission-driven culture.

Qualifications

• CPR certification current and maintainable.
• Valid driver’s license with reliable transportation.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Deliver attentive, one-on-one support to patients and their families.
• Conduct assessments of physical, emotional, and spiritual needs.
• Monitor vital signs and document accurately in charting.
• Administer physician-ordered medications and treatments with empathy.
• Educate families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
