Job summary

Central Care Incorporated is seeking a Registered Nurse Case Manager to join the team at Cannon Air Force Base in New Mexico. You will provide nursing care and case management for active-duty service members, retirees, and their families, ensuring quality, continuity, and coordination of care.

Ideal candidates hold an Associate’s Degree of Nursing, current unrestricted RN licensure, and at least three years of RN experience within the last four years.

Qualifications

• Associates Degree or higher in nursing required.
• U.S. Citizen required.
• RN license in good standing in a State, Territory, Commonwealth, or DC required.
• Three years of RN experience within the last four years required.
• Must possess one of CCM, CDMS, CRRN, COHN, CRC, RN-NCM, CMC certifications.

Responsibilities

• Participate in all phases of the Case Management Program (CMP) and ensure CMP standards of care.
• Provide nursing expertise about the CM process: assessment, planning, implementation, coordination, and monitoring.
• Develop and implement local CM strategies across inpatient, outpatient, onsite and telephonic care.
• Develop tools to support case management and integrate CM with utilization management and social work.
• Maintain liaison with community agencies and accurately document patient care data.
• Develop and implement treatment plans to ensure continuity of care toward optimal wellness.

Position Type: Full-Time | Civilian Contractor via Central Care Incorporated

Compensation & Benefits

• $53.50/hour Base Rate

• $5.09/hour Health & Welfare (H&W)

• 80 Hours PTO (accrued annually based on wage determination)

• 56 Hours Sick Leave (accrued annually)

• 11 Paid Federal Holidays

Schedule: Monday -Friday, with 8-hour shifts between 7:00 AM-5:00 PM, not to exceed more than 40 hours per week.

Minimum Qualifications:

• Degree/Education: Associates Degree or higher
• Must be a U.S. Citizen
• Degree: Associate’s Degree of Nursing.
• Experience One year of experience within the last two years as a registered nurse
• Licensure: RN License -Must hold a full, current, and unrestricted license in a State,
• Territory, Commonwealth, or the District of Columbia.

Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).

Experience:

• RN shall have experience in providing direct patient care, patient and staff education, administration, and/or research.

Certifications:

• Basic Life Support
• MUST Possess ONE of the Following Certifications:
• Commission for Case Manager Certification Certified Case Manager (CCM)
• Certified Disability Management Specialist (CDMS)
• Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse(CRRN)
• American Board for Occupational Health Nurses Certified Occupational Health
• Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S).
• National Board for Certification in Continuity of Care: Advanced Certification in
• Continuity of Care (ACCC)
• Commission on Rehabilitation Counselor Certification: Certified Rehabilitation
• Counselor (CRC)
• American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
• National Academy of Certified Care Managers: Care Manager Certified (CMC)

Experience:

• Current experience requirement is three years of in the last four years as a registered nurse case manager.

Core Duties:

• Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
• Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
• Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM
• Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
• Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.
• Maintain liaison with appropriate community agencies and organizations.
• Accurately collect and document patient care data.
• Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
• Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
• Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
