Paramount Home Health is seeking a compassionate, organized, and clinically skilled Registered Nurse (RN) to join our team as a Nurse Case Manager. This position is responsible for coordinating and managing the delivery of home health services for patients residing within Paramount Assisted Living. The Nurse Case Manager conducts patient assessments, develops and implements individualized care plans, performs skilled nursing visits, coordinates interdisciplinary services, and ensures compliance with all applicable regulatory and documentation requirements.

Essential Duties and Responsibilities
Clinical Care Management
- Perform comprehensive patient assessments and reassessments to determine healthcare needs and eligibility for home health services.
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge assessments as appropriate.
- Develop, implement, and monitor individualized patient care plans in collaboration with physicians, patients, families, and interdisciplinary team members.
- Provide skilled nursing services in accordance with physician orders and agency policies.
- Assess patient progress and modify care plans to promote optimal clinical outcomes.
- Monitor and manage chronic disease conditions, medication regimens, and overall patient health status.
Case Management
- Serve as the primary clinical coordinator for assigned patients.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated care delivery.
- Identify patient needs and facilitate appropriate referrals to community resources and support services.
- Educate patients and families regarding disease management, medications, treatment plans, safety measures, and health maintenance.
- Advocate for patients to ensure quality, cost-effective, and patient-centered care.
Documentation & Compliance
- Accurately document patient assessments, care provided, and patient outcomes in Brightree and other required systems.
- Complete visit documentation within 24 hours of each patient encounter.
- Ensure compliance with Medicare, Medicaid, state regulations, agency policies, and accreditation standards.
- Participate in quality assurance and performance improvement initiatives.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.
- Maintain effective communication with patients, families, physicians, and interdisciplinary team members.
- Participate in care conferences, team meetings, and case reviews.
- Provide timely updates regarding changes in patient condition or care needs.
- Support a collaborative and patient-focused care environment.
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license in the Commonwealth of Pennsylvania.
- Graduate of an accredited school of nursing.
- Minimum one (1) year of professional nursing experience.
- Current CPR/BLS certification.
- Strong assessment, clinical decision-making, and organizational skills.
- Excellent verbal and written communication abilities.
- Proficiency in electronic medical records and healthcare documentation systems.
- Reliable transportation and proof of automobile insurance.
Preferred Qualifications
- One (1) or more years of Home Health Nursing experience.
- Experience serving as a case manager in a home health, hospice, long-term care, assisted living, or community health setting.
- Knowledge and experience with OASIS assessments.
- Experience utilizing Brightree or similar home health software platforms.
- Familiarity with Medicare Conditions of Participation and home health regulations.

- Knowledge of home health regulations, standards of practice, and quality measures.

Working Conditions
- Primarily works within Paramount Assisted Living in the Pittsburgh area.
- Minimal travel requirements; all patient care responsibilities are confined to a single location.
- Frequent standing, walking, bending, lifting, and patient care activities.
