The RN Care Manager (RN CM) performs utilization management, case management, and discharge planning activities for acute care and extended care patients, including ambulatory and emergency department patients. The RN Care Manager collaborates with healthcare teams to assess patients' transition/discharge needs, prevent unnecessary admissions/readmissions, and facilitate timely transitions/discharges.
If hired after January 01, 2016, a Bachelor of Science in Nursing from an accredited institution is required. A master's degree is preferred. Must hold and maintain a Rhode Island Registered Nurse license. A minimum of 3 to 5 years of related clinical experience as a Registered Nurse is required. Case Management experience or knowledge is required, demonstrated by either a Nursing Care Management Certificate of Graduate Studies or current knowledge of Care Management through a pre-hire assessment. Certification in Case Management (CCM or ACM) must be earned within 24 months of employment. The ability to communicate effectively with residents and physicians, interpret medical data, and influence interdisciplinary teams is essential. Knowledge of utilization management, quality management, and third-party payers is preferred. Proficiency with multiple computer systems (EMR, Microsoft Office, etc.) is required. Participation in weekend and holiday coverage rotation is expected, with flexibility to cover as needed.
