The Care Manager is responsible for providing care management to a group of health plan members, creating the Person-Centered Service Plan (PCSP), coordinating services, and documenting care in accordance with health plan standards and under the guidance of the Care Management Supervisor.

Performance Responsibilities and Standards:

  1. Provide care management for members through an interdisciplinary approach, including assessment, planning, facilitation, and advocacy to meet members' needs.
  2. Implement an appropriate, quality, and cost-effective plan of care utilizing all available resources, collaborating with members, families, healthcare teams, providers, and community programs based on the UAS-NY assessment.
  3. Oversee caseload of managed care members.
  4. Perform all care management activities in compliance with regulatory requirements; complete clear, concise, and timely documentation; coordinate support staff including LPNs, Community Health Workers, and Care Coordinators; provide guidance and education for team-based patient management.
  5. Participate in departmental meetings and attend all scheduled staff training and meetings.
  6. Adhere to all privacy regulations, including HIPAA, and organizational policies regarding confidentiality, privacy, and security.
  7. Communicate effectively with members, families, health plan staff, and providers.
  8. Act as a liaison and advocate for members.
  9. Demonstrate cultural competence by respecting members' cultural health beliefs, language needs, and support systems.
  10. Other duties as assigned.

Requirements:

  1. Nursing: Current NYS RN license and registration; BSN preferred, associate degree required; minimum one year of nursing experience.
  2. Experience in long-term care and/or managed care is a plus.
  3. Knowledge of homecare, Medicare, and Medicaid is a plus.
  4. Care Management Certification is a plus.
  5. Bilingual skills are a plus.
  6. Ability to work independently, prioritize workload.
  7. Proficiency in basic computer skills and Microsoft Office.
  8. Strong communication skills, both in person, virtually, and via phone.
  9. Valid NYS driver’s license and ability to drive in all weather conditions.
  10. Ability to learn and utilize organizational software systems routinely.

Physical Requirements:

  • Work from home with visits to members’ homes; attendance at in-office meetings may be required.
  • Postures include standing, sitting, bending, walking, and driving.
  • Frequent use of a laptop for entering orders and information.
  • Sensory requirements include speech, vision, smell, and hearing.
  • Lifting up to 20 pounds frequently and up to 50 pounds occasionally.
  • Bloodborne Pathogens Exposure Category I: tasks involve potential exposure to blood, body fluids, or tissues.

Compensation and Benefits:

  • Salary: $72,415.71 - $83,922.76 annually, with a potential sign-on bonus of up to $15,000 (restrictions may apply).
