The Care Manager is responsible for providing care management to a group of health plan members, creating the Person-Centered Service Plan (PCSP), coordinating services, and documenting care in accordance with health plan standards under the guidance of the Care Management Supervisor.

Performance Responsibilities and Standards:

  1. Provide care management for members through assessment, planning, facilitation, and advocacy to meet their needs.
  2. Implement a quality, cost-effective plan of care utilizing all available resources, collaborating with members, families, healthcare teams, and community programs based on the UAS-NY assessment.
  3. Oversee caseloads of managed care members.
  4. Perform all activities in compliance with regulatory requirements, ensuring documentation is clear, concise, and timely. Coordinate support staff including LPNs, Community Health Workers, and Care Coordinators, providing guidance and education for efficient team management.
  5. Participate in departmental meetings, staff training, and adhere to all policies and regulations regarding confidentiality and privacy, including HIPAA.
  6. Communicate effectively with members, families, healthcare staff, and providers. Act as a liaison and advocate for members.
  7. Demonstrate cultural competence by respecting members' cultural health beliefs, language needs, and support systems.
  8. Perform other duties as assigned.

Requirements:

  1. Licensed and registered as a Registered Professional Nurse in New York State. BSN preferred; associate degree required. Minimum one year of nursing experience.
  2. Experience in long-term or managed care is a plus.
  3. Knowledge of homecare, Medicare, and Medicaid is advantageous.
  4. Care Management Certification is a plus.
  5. Bilingual skills are a plus.
  6. Ability to work independently, prioritize tasks, and be computer literate, with knowledge of Microsoft Office.
  7. Strong communication skills, both in person and virtually.
  8. Valid NYS driver’s license and ability to drive in all weather conditions.
  9. Ability to learn and use organizational software systems routinely.

Physical Requirements:

  • Work from home with visits to members’ homes; in-office attendance may be required.
  • Postures include standing, sitting, bending, walking, and driving.
  • Use of a laptop computer frequently.
  • Sensory requirements include speech, vision, smell, and hearing.
  • Lifting up to 20 pounds frequently and up to 50 pounds occasionally.
  • Potential exposure to blood, body fluids, or tissues.

Salary and Benefits:

  • Salary range: $72,415.71 - $83,922.76 annually.
  • $15,000 sign-on bonus (restrictions may apply).
  • 401k with employer match, tuition reimbursement for BSN after 6 months, no rotating shifts, holidays, or weekends, partially funded HSA, employee recognition, paid time off, holidays, sick leave, disability, EAP, and more.
