Job summary

UVA Health’s Ehlers-Danlos Center is seeking an RN Care Coordinator to support patients with Ehlers-Danlos Syndrome through assessment, care coordination and ongoing communication. You will work closely with providers and a multidisciplinary team in an ambulatory clinic to organize visits and connect them to the broader plan of care.

The role is onsite, Monday–Friday 8:00 a.m. to 4:30 p.m., with no nights, weekends, or holidays.

Qualifications

• Graduate of an accredited nursing program.
• BSN required within 5 years of hire.
• 1 year of relevant nursing experience.
• Licensed Registered Nurse in Virginia; AHA BLS certification.

Responsibilities

• Perform comprehensive intake and document medical history, meds, allergies and social history.
• Synthesize patient information and present to providers.
• Conduct telephone triage for symptoms and care concerns.
• Route prescription refills and clinical advice requests.
• Manage patient communications via Epic and MyChart.
• Review charts before appointments and ensure required documentation is complete.
• Support patient flow, clinic prep and throughput.
• Coordinate referrals to cardiology, genetics, GI, PT/OT, behavioral health, nutrition, ophthalmology.

Job description

Bring your ambulatory nursing expertise to a rapidly expanding specialty program where clinical judgment and coordinated care directly shape the patient experience.

As an RN with UVA Health’s Ehlers-Danlos Center, you will support patients with Ehlers-Danlos Syndrome and related hypermobility disorders through comprehensive assessment, care coordination, and ongoing communication. This is an opportunity to join a developing multidisciplinary program, build expertise in a highly specialized area of medicine, and help shape the future of a growing medical specialty .

The Role

You will serve as a clinical resource within an ambulatory clinic caring for patients whose needs may span multiple specialties. Working closely with providers, an RN Care Coordinator, an LPN, advanced practice providers, and other clinical partners, you will help ensure that each visit is organized, clinically informed, and connected to the patient’s broader plan of care.

The position is fully onsite and follows a Monday through Friday, 8:00 a.m. to 4:30 p.m. clinic schedule , with the possibility of slight adjustments based on coverage needs. There are no night, weekend, or holiday requirements.

An average day may include:

• Completing patient intake and reviewing medications, allergies, social history, and relevant medical history

• Synthesizing and presenting pertinent patient information to providers

• Conducting telephone triage for symptoms and patient-care concerns

• Triaging prescription refill and clinical advice requests, then routing them appropriately

• Managing patient communications through Epic and MyChart

• Reviewing charts before appointments and confirming that required documentation and testing are available

• Supporting patient flow, clinic preparation, and efficient throughput

• Coordinating referrals to disciplines such as cardiology, genetics, gastroenterology, physical therapy, occupational therapy, behavioral health, nutrition, and ophthalmology

• Partnering with the RN Care Coordinator to support continuity across the patient’s care journey

This role is well suited for an RN who recognizes subtle clinical patterns, communicates clearly, and can balance direct patient interaction with detailed clinical coordination. Pediatric, general medicine, behavioral health, ambulatory, or complex-care experience can be especially valuable because the center may care for patients beginning in early adolescence.

MINIMUM REQUIREMENTS

Education : Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.

Experience : 1 year of relevant experience.

Licensure : Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Healthcare Provider BLS certification required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease

The pay range for this role is $38.66 - $50.97 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
