Job summary

Trinity Services, Inc. is seeking a Registered Nurse (RN) in New Lenox, IL to promote the health of individuals with disabilities and mental illness. Under the Network Director, you’ll oversee medical services, including medication administration and regulatory compliance.

You will collaborate with the support team, educate staff, and coordinate care across physicians and hospitals. Full-time, M-F with on-call duties, with a salary of $73,000 annually.

Qualifications

• Must hold ADN and an active IL RN license.
• Completion of DHS Nurse-Trainer Training Program.
• Prefer 1 year nursing experience with disability/mental health exposure.

Responsibilities

• Conduct medical reviews and assessments (HRS, nursing assessments).
• Coordinate follow-up care after appointments, labs, or hospital visits.
• Educate individuals and staff on health conditions and practices; respond to urgent inquiries.
• Delegate medication administration to non-licensed personnel where appropriate.
• Maintain POS and MARs and ensure regulatory compliance.
• Oversee medical transitions during admissions or discharges.
