Job summary

Responsibilities

• Ensure accuracy of MDS coding to maximize reimbursement.
• Ensure accuracy of clinical documentation for resident care.
• Serve as Nursing Educator to train staff on documentation.
• Act as a key member of the nursing leadership team to improve patient outcomes.
• Coordinate case management with managed care to optimize therapies and nursing services.
• Maintain clinical and financial records and share with payers.
• Collaborate with social workers on care conferences.

Qualifications

Licensed RN in the state of PA

2 years RNAC experience in a long-term care setting

The Registered Nurse Assessment Coordinator (RNAC) would:

• Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care (CMI).
• Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care.
• Nursing Educator: – Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough. This requires high levels of in-person interfacing with both residents and employees.
• Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes. This includes engagement in leadership standups and influence within interdisciplinary teams.
• Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes)
• Manage clinical and financial record keeping and dissemination of these records to appropriate payers.
• Take joint ownership of care conferences with social workers
• Perform other duties as required.
