Job summary

• Specialized certifications as needed for ambulatory oncology.

Responsibilities

• Assess the patient's physiologic health status through interviews and health assessments.
• Evaluate psychosocial health and identify cultural/ethnic needs.
• Develop criteria for outcome measurement and set individualized goals.
• Formulate the plan of care and coordinate with the healthcare team.
• Implement the plan safely and communicate with patients and families.
• Evaluate care outcomes and document effectiveness.
• Collaborate with care team to plan and execute treatment regimens.
• Provide wound care and manage wounds (Stages I–IV).
• Administer medications and educate patients on drug interactions.
• Engage in patient and family education on health management.

About This Role

What You'll Do

• Assess the patient's physiologic health status through interviews and comprehensive health assessments.
• Evaluate patient's psychosocial health status and identify cultural/ethnic requirements.
• Develop criteria for measurement of patient outcomes and establish individualized patient goals.
• Formulate and document the plan of care, coordinating with healthcare team members.
• Implement the plan of care, ensuring safety precautions and effective communication with patients and families.
• Evaluate care provided for patient outcomes and document effectiveness.
• Collaborate with care team members in planning and executing treatment regimens.
• Provide wound care and manage pressure ulcers (Stages I-IV).
• Administer medications correctly and educate patients on drug interactions.
• Participate in patient and family education, ensuring understanding of health management.

Requirements
