Amedisys, Inc. is seeking a Hospice Admissions Registered Nurse for the 1st shift on a full-time basis.

The role focuses on coordinating admissions, assessing initial patient needs, and partnering with the attending physician and interdisciplinary team to establish a plan of care at the patient’s home. Responsibilities include providing skilled interventions, documenting progress, and serving as a liaison among disciplines while supporting families through the admissions process.

Formación

• Current RN license, specific to the state you are assigned to work.
• One year of clinical RN experience.
• Current CPR certification.

Responsabilidades

• Provides skilled interventions and instructions to patient and caregivers for comfort.
• Acts as a liaison between disciplines and facilities during the admission.
• Maintains individualized patient care progress records and submits documentation.
• Records pain/symptom management changes and outcomes as appropriate.
• Communicates findings and changes to the clinical manager/physician and team.
• Performs on-call responsibilities as assigned.

• $95,000.00 - $98,000.00

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical RN experience.
• Current CPR certification.
