Qualifications

• Two (2) years of experience as a registered nurse.

Responsibilities

• Collects data and completes nursing assessment using area/age appropriate standards and tools.
• Develops and updates patient care plans in collaboration with patient/family and healthcare team.
• Sets realistic goals and outcomes for discharge and coordinates transitions of care.
• Provides direct nursing care within the established plan and delegates tasks as appropriate.
• Maintains patient safety and infection control, documents progress and communicates changes to the team.
• Participates in quality improvement efforts and evaluates patient responses to interventions.
