Job summary

Yale New Haven Health is actively hiring Registered Nurses for Adult Acute Care Medicine units at the Saint Raphael Campus. Responsibilities include assessing patients, planning and coordinating care, educating patients and families, and supervising staff.

Qualifications

• BSN preferred; graduate from an accredited nursing program.
• Valid State of Connecticut Registered Nurse License.
• BLS required.

Responsibilities

• Assess patient's physical and psychosocial condition on an ongoing basis.
• Develop, document, and communicate a plan of care with patient, family and healthcare team.
• Implement and coordinate the plan of care and evaluate patient response and readiness for discharge.
• Provide education to patients and families and document outcomes.
• Supervise non-RN personnel on the unit and act as charge nurse when needed.
• Perform other duties as assigned to ensure smooth operation of the department.
