Job summary

Memorial Hospital and Health Care Center is seeking an RN Case Manager to drive patient journeys through the healthcare system. You will coordinate care with physicians, nurses, therapists, social workers, patients, and families to remove barriers and ensure safe transitions throughout the continuum of care.

Qualifications

• Active Indiana/Kentucky RN license or compact RN license.
• 2 years of acute RN experience.

Responsibilities

• Coordinate patients' journey through the healthcare system with physicians, nurses, therapists, social workers, patients and families.
• Help remove barriers, coordinate services, and create safe, successful transitions across the continuum of care.
