The CARIN Alliance is seeking an RN Complex Care Coordinator in Austell, GA to assess and coordinate care for complex patients and ensure safe, timely discharge planning across care settings.
You will conduct comprehensive clinical and psychosocial assessments, manage high-risk caseloads, collaborate with interdisciplinary teams, and guide patients and families through decision-making while connecting them with community resources.
The RN Complex Care Coordinator is responsible for assessing and coordinating care for complex patients, ensuring safe and timely discharge plans across the continuum of care.
Key duties include conducting comprehensive clinical and psychosocial assessments, managing caseloads of high-risk patients, and collaborating with interdisciplinary teams to develop personalized discharge strategies.
They serve as a resource on complex social determinants, end-of-life care, and community resources, guiding patients and families through decision-making processes.
Additional responsibilities involve facilitating family meetings, advocating for patients, and supporting community agency negotiations.
Qualifications include a nursing degree (Associate, Diploma, or Bachelor’s preferred), RN licensure (single or multi-state), and certifications such as BLS.
Minimum of 3 years healthcare experience, with at least 2 years in acute care care coordination, is required.
Must possess excellent communication, assessment, organizational, and problem-solving skills, along with knowledge of regulations and community resources.
