Job summary

Southwest Network seeks a Registered Nurse to join our Assertive Community Treatment (ACT) Team in Phoenix, AZ. You will deliver nursing services, engage clients, perform assessments, triage, and support treatment planning.

The role includes medication administration, health education, and advocacy, with opportunities to contribute to utilization management and quality improvement initiatives. Competitive benefits and a stable practice environment await.

Qualifications

  • BSN degree from an accredited program or equivalent experience.
  • Current Arizona RN license.
  • Minimum one year psychiatric/behavioral health experience with seriously mentally ill adults; ACT experience preferred.
  • Must maintain CPR, First Aid, and Therapeutic Options certifications.
  • Must have valid Arizona driver's license and car insurance.
  • Must be at least 21 years of age.

Responsibilities

  • Provide nursing services to members on the Assertive Community Treatment Team.
  • Engage, assess, triage and participate in treatment planning.
  • Administer medications and provide health education and advocacy.
  • Contribute to utilization management and quality improvement activities.

Provides nursing services to members on our Assertive Community Treatment Team. Services include engagement, assessment, triage, treatment planning, medication administration, health and medication education, physical healthcare advocacy, and supportive care. Participates in utilization management and quality improvement activities as required.

REQUIREMENTS
Education, Certification, and Experience Requirements
  • Bachelor of science degree in nursing from an accredited program or equivalent combination of education, training, and/or experience.
  • Current R.N. license issued by Arizona Board of Nursing
  • Minimum one (1) year prior psychiatric/behavioral health experience with seriously mentally ill adults required; previous experience working on an ACT team or equivalent experience preferred.
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)
  • Must maintain a valid Arizona driver's license and car insurance
  • Must be at least 21 years of age.
