Job summary

Southwest Network is seeking an ACT case manager to deliver assertive treatment services in community settings. You will coordinate ISPs, discharge plans, and crisis interventions while advocating for members and ensuring continuity of care.

This role includes after-hours coverage and participation in ongoing training. A valid driver’s license and relevant behavioral health experience are required, with opportunities for professional growth within the organization.

Qualifications

  • Must have a behavioral health or related degree at various levels (MS/BS preferred).
  • 1 year related experience with a non-related BA/BS is acceptable.
  • Associate degree plus 1 year related experience acceptable.
  • High school diploma with 3 years related experience accepted.

Responsibilities

  • Provide assertive community treatment services and direct services in a community setting.
  • Develop individualized service plans (ISPs) and coordinate services and discharge planning.
  • Act as member advocate and coordinate crisis intervention and services.
  • Provide after-hours care and on-call coverage for ACT team.
  • Assist in crisis intervention and support to all members served by ACT.

Job description

As a member of an Assertive Community Treatment (ACT) team, functions as a case manager with primary responsibility for providing assertive treatment services, implementing plans for treatment, rehabilitation and support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Acts as a member advocate, coordinates services and discharge plans, and assists in crisis intervention. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.

All members of our team are expected to provide services in accordance with the Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services.

Education, Certification, And Experience Requirements
  • Must possess one of the following:
    • Master's or bachelor's degree with a behavioral health or related field
    • Bachelor's degree in a non-related field PLUS one year related experience
    • Associate's degree in any field PLUS one year related experience
    • High school diploma or equivalency certificate PLUS three years' related experience
  • Minimum (1) one year working with seriously mentally ill adults required; previous experience working on an ACT team or equivalent experience preferred.
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)
  • Must maintain a valid Arizona driver's license and car insurance
  • Must be at least 21 years of age.
Knowledge, Ability, And Skills
  • Must have the ability to maintain excellent member and family relationships and provide excellent customer service
  • Requires effective written, verbal and interpersonal communication skills
  • Ability to stay organized and have critical thinking, time management and problem solving skills are a must
  • Ability to utilize multiple types of office equipment (i.e. phones, copier, fax, etc.)
  • Ability to type 25 to 40 WPM is preferred
  • Must be proficient in Microsoft Office software
  • Must be able to utilize the internet and various web browsers
