Job summary

Regency Integrated Health Services' Harlingen Nursing and Rehabilitation Center is seeking an RN to provide patient-centered care on an as-needed (PRN) basis. You will assess residents, implement care plans, administer medications, and document visits in clinical records.

The role requires active Texas RN licensure and completion of an accredited RN program; SNF experience is preferred. You will collaborate with physicians, nurses, and care teams to ensure quality outcomes and compliant care

Qualifications

• Must have completed RN program at an accredited institution.
• Active Registered Nurse license in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Assure implementation of care plans as directed.
• Administers medications as prescribed and records visits in the clinical records.
• Conduct rounds with physician and document visits.
• Perform narcotic reconciliation with incoming/outgoing shifts.
• Identify significant changes in resident condition and take action per policies.

Essential Functions

• During rounds observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conducts rounds with physician and records visits in clinical records.
• Performs narcotic reconciliation with incoming and outgoing shifts.
• Identifies significant changes in the condition of residents and takes necessary action per facility policies and procedures.
• Oversees and monitors aides.
• Initiates and completes Incident and Accident Reports per facility policy.
• Discharges residents per facility policy.
• Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
• Communicates with resident’s point of contact when they ask for a status update or if there is a change in condition.
• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator.

Qualifications

Educational/Training Requirements: Has completed a RN program at an accredited vocational school, college, or university.

Licensing Requirements: Must be an active Registered Nurse in the state of Texas in good standing.

Experience Requirements: Experience working in a skilled nursing facility is preferred.
