Job summary

Oak Street Health is hiring a Registered Nurse for our Durham, NC clinic. The role focuses on coordinating patient care, delivering competent nursing services, and guiding patients and their families through treatment plans.

The position requires NC RN licensure, CPR/BLS, EMR experience, and strong communication. It offers a $5,000 sign-on bonus and comprehensive benefits. 40 hours per week, full-time, with CVS Health bonus eligibility.

Qualifications

• Active RN license in North Carolina in good standing.
• CPR and/or BLS certification.
• Electronic Medical Record (EMR) experience.
• Clinic setting with hands-on patient care, injections and vaccinations.
• Maintain patient confidentiality and HIPAA compliance.
• Strong communication and assessment skills.
• Ability to multitask and work independently.
• Collaborative with interdisciplinary team.

Responsibilities

• Provide competent nursing care safely and consistently.
• Respond to telephonic requests and be reliable for patients.
• Triage and manage symptoms, in person or remotely.
• Use protocols for medication management and prior authorizations.
• Review and update patient medications as needed.
• Educate patients and provide direct care, esp. chronic conditions.
• Collaborate with care team to monitor and progress care plans.
• Build relationships with patients and caregivers for preventive care.
• Create welcoming environment and address equity of care.
• Participate in care team meetings and daily huddles.
• Deliver exceptional patient experience through service and respect.
• Perform point-of-care testing and phlebotomy as needed.
