Job summary

Rocky Mountain Care Home Health & Hospice in Vernal, Utah is seeking a compassionate Registered Nurse (RN) Case Manager to join our growing team. This is a full-time role focused on delivering personalized in-home care and coordinating with physicians and caregivers.

You will assess patient needs, develop care plans, administer treatments, and provide education to patients and families. A current RN license and hospice or home health experience are preferred.

Qualifications

• Current, unrestricted RN license.
• Hospice or Home Health experience preferred.
• Home Health experience accepted.
• Excellent communication and organizational skills.

Responsibilities

• Assess and monitor patient conditions and changing care needs.
• Develop, implement, and update individualized care plans.
• Administer medications and treatments as ordered.
• Provide skilled nursing care, including wound care and IV therapy.
• Collaborate with physicians and the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
• Coordinate patient care and communicate changes in condition.
• Provide compassionate support and education to patients and families.
