Job summary

Rocky Mountain Care Home Health & Hospice in Vernal, Utah is seeking a compassionate Registered Nurse (RN) Case Manager to join our growing team. This is a full-time home health role with competitive hourly pay and mileage/cell phone reimbursements.

You will assess patient needs, develop care plans, administer medications, provide skilled nursing including wound care and IV therapy, and collaborate with physicians and the interdisciplinary team to ensure quality care at patients' homes.

Qualifications

• Active RN license in good standing.
• Hospice experience preferred; home health experience accepted.
• Strong clinical judgment and assessment skills.
• Excellent communication and organizational abilities.
• Ability to work independently while collaborating with a care team.

Responsibilities

• Assess and monitor patient conditions and changing care needs.
• Develop, implement, and update individualized care plans.
• Administer medications and treatments as ordered.
• Provide skilled nursing care, including wound care and IV therapy.
• Collaborate with physicians and the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
• Coordinate patient care and communicate changes in condition.
• Provide education and support to patients and families.
