Job summary

Trinity Health PACE is seeking a Registered Nurse for the PACE Kinder Park location. This full-time role oversees skilled nursing care, develops care plans, and coordinates care with the interdisciplinary team to optimize participant health.

Hours are 8am–4:30pm, Monday–Friday, with full-time benefits. The RN will provide prescribed treatments, educate patients and families, maintain documentation, and ensure compliance with nursing standards.

Qualifications

• Graduate of an accredited Nursing program with an active RN license.
• Minimum of 2 years experience with geriatric population.
• CPR certification required.
• Valid driver’s license and proof of insurance.
• Strong communication skills and proficiency in Microsoft Office.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Provide skilled nursing care, including administering treatments and medications as prescribed.
• Conduct assessments, develop care plans, and collaborate with the IDT to monitor participant health.
• Educate participants and families on self-care, medication use, and health management.
• Ensure adherence to professional nursing standards and regulatory requirements.

Position Purpose:
The Registered Nurse (RN) at the PACE Organization is responsible for providing skilled nursing care, prescribing treatments, and developing care plans for participants. This role oversees service delivery and ensures the coordination of care to optimize participant health and well-being.

What You Will Do:
