Trinity Health PACE in Massachusetts is seeking a Part-time Nurse Practitioner to deliver primary medical management in collaboration with PACE physicians. You will assess patients, develop care plans, and ensure comprehensive, patient-centered care.
The position includes discharge planning, treatment of acute illnesses, home visits, and participation in policy development and on-call rotations. Requires NP with prescriptive authority and BLS; CDS/DEA and ANCC/AANP board preferred.
