As a Hospice RN/ RN Case Manager, you will:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager and will maintain primary control and professional management of each patient.
  • Act as the primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while fostering a trusting and compassionate relationship.
  • Empower patient’s caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary group including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
  • Evaluate patient and family needs and assist in the development and execution of the care plan for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
About You

Hospice Registered Nurse / RN Case Manager Requirements:

  • Have a compassionate, kind, and empathetic heart for your patients and their families.
  • Be comfortable providing care wherever the patient calls “home”.
  • Demonstrate flexibility and composure while supporting others in a time of crisis.
  • Hold an RN license in the state of employment (or be eligible to obtain one).
  • Have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology (preferred).
  • Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, critical care, or related fields is beneficial.
  • Possess a current driver’s license and automobile insurance.
  • Maintain current CPR certification.
