The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with applicable laws, regulations, and policies.

Responsibilities include:

• Coordinating care with healthcare professionals and maintaining continuity of care through planning and communication.
• Conducting initial and comprehensive nursing evaluations, developing care plans, and documenting accurately and timely in collaboration with physicians.
• Performing patient assessments and developing appropriate care plans.
• Ensuring patients meet home health eligibility and medical necessity guidelines.

License Requirements:
