Job summary

Angels Care Home Health in Cottonwood, AZ seeks a compassionate Registered Nurse Case Manager to coordinate all aspects of patients' home care. You will perform admission visits, assess eligibility for home health services, and develop individualized plans of care in collaboration with the medical team.

The ideal candidate is a graduate of an accredited nursing program, holds an active RN license, has a valid driver's license, and enjoys direct patient care.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state.
• Valid driver’s license and reliable transportation.
• CPR certification and Hepatitis consent/declination.
• One year of RN experience; two years preferred.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to the patient in the home.
• Determine eligibility for home care services and develop the plan of care.
• Interact with other medical disciplines as needed.
