The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and policies.

Responsibilities include:

• Coordinating the care plan with healthcare professionals and ensuring continuity of care.
• Performing initial and comprehensive nursing evaluations, developing care plans, and documenting accurately and timely.
• Conducting patient assessments and developing appropriate care plans.
• Ensuring patients meet home health eligibility and medical necessity guidelines.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to transportation

Additional State Requirements:

• LA: At least one year of clinical experience as an RN; experience as an LPN may be considered.
• Other states: No additional specific requirements.
