Job summary

Entyre Care is seeking an experienced RN to lead home-based care for elders and adults with disabilities across our service area. You will perform initial assessments, develop personalized care plans, coordinate with families and primary care providers, and document care in patient records.

This full-time hybrid role requires travel by car to client homes (3–5 visits daily, as needed). You’ll train and supervise caregivers, ensure regulatory compliance, and leverage standard software to manage

  • Active RN license in the state of employment.
  • Minimum 2 years direct elder care or disability care experience.
  • Ability to travel within assigned geographic area; valid driver’s license and automobile insurance.
  • Experience in case management, care planning and assessment for cases including eldercare and other complex medical needs.
  • Proficiency in common software and web-based applications (MS Office, case management systems).
  • Experience with MDS assessment tool is a plus.
  • Assess client status on initial home visit to identify needs and develop a care plan.
  • Teach and counsel patient, family and caregiver in meeting nursing needs.
  • Document accurately; maintain a clinical record for each patient.
  • Complete the MDS-HC and other nursing assessments; coordinate other assessments.
  • Training, evaluating, and supervising caregivers.
  • Monitor health status of clients to ensure care delivery.
  • Proficiency with Outlook, Excel, Word, PowerPoint, and Teams.
  • Additional state-specific qualifications may be required.

This role frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).

What You’ll Bring
  • Must hold an active Registered Nurse (RN) License in the state of employment

  • Minimum two (2) years of experience in direct care of elders or adults with disabilities

  • Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required

  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting

  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc)

  • Experience with Minimum Data Set (MDS) assessment tool a plus.

  • Assess client status upon initial home visit to identify needs in order to develop a comprehensive case management plan.

  • Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting nursing and related needs.

  • Document in an accurate and timely manner. Maintain a clinical record for each morbidity patient.

  • Completing the MDS-HC and other nursing assessments while coordinating all other applicable clinical assessments.

  • Training, evaluating, and supervising caregivers.

  • Monitoring the health status of all members to ensure that all needed care is properly delivered.

  • Proficiency in common software and web-based applications including Microsoft Outlook, Excel, Word, PowerPoint, and Teams.

  • Additional state-specific qualifications may be required.
